1BDE MEDDET believes that health is a vital component of a comprehensive Wellness Program. It is every soldier’s responsibility to maintain his or her health so that in a time of need, they will be ready to SERVE.
GETTING STARTED:
Complete the GaSDF Medical Profile Assessment Form (600-63A), which can be found on line _________________ and turn it into your commanding officer, and he in turn will send the form to 1BDE MEDDET for review.
Update and/or complete the MEDDET’s Health History form, which can be found online at: http://www.meddet.com/recruitment.php.
This form may be given to the unit’s embedded medic for review or forwarded to the 1BDE MEDDET for review.

After confidential review, by the medical officers in 1BDE MEDDET, of the health information provided in the above listed forms, a duty level will be assigned to each soldier based on their current health status.

Routine check-ups with a healthcare professional are an important part of staying healthy. Determine with your personal healthcare provider what that schedule should be for you based upon age and health status.
